• Passport size photographs
• Valid mobile number & email ID
• Bhamashah card ki photocopy
• Front Page photocopy of Bank Passbook.
• Undertaking for the related scheme
• BPL card copy ( if any)
• Death Certificate in case of death
• FIR or Postmortem report copy
• Disability certificate in case of death
• Child’s birth certificate for Maternity scheme
• Date of birth proof
