Apply Labour Welfare Fund Spectacles Scheme

Step1:

Visit the official site of the Haryana Labour welfare board (https://hrylabour.gov.in/). Choose Welfare board beneficiary from the user select type dropdown menu. Enter your username (of seven letters first three words your name and last five digits of your mobile number) and password and captcha and log in. Click here to know how to generate a username and password.

Step 2:

Go to option “Scheme” as showing on the menu bar. Click on the “View Details” option in front of the said scheme name. Scroll down and click “Apply Scheme”.

Step 3:

Your Name, Father Name, Aadhar Number will be shown.

Step 4:

Then You will have to upload the required certificates self-attested and attested through your employer. Please Upload all certificates/ documents in a jpeg, png, jpg, pdf, doc, Docx file max 500KB size only

1. Medical Certificate: Medical certificate from any MD/MS/MBBS Doctor or from any Govt Hospital to justify the requirement for Spectacles or Sunglasses.
2. Bill/Ticket Receipt: Scan copy of the original bill of purchased spectacles as prescribed by the Doctor.
3. Ration Card: Upload a Scanned copy of your family ration card.
4. Employee Salary Slip: Upload a Scanned copy of your salary slip signed by your employer.
5. Employee and Employers Certificate: Employee and Employers Certificate can be downloaded from (Here). Take and Printout fills up and gets it attested by yourself and your employer. Same, the undertaking can be downloaded from (here). Take and Printout fills up and gets it attested by yourself and your employer.

Step 6:

After Uploading all documents, Review them and Click on generate OTP. An OTP will be sent on your Mobile number seeded with the Aadhar number. Enter That OTP and apply.
YOu can successfully apply for the scheme now wait and check the scheme status after a few days. You will receive the scheme amount in your bank account.
